Zebra Telehealth Refund Policy

To request an appointment with Zebra Telehealth, a valid payment method is required. A transaction hold will be placed on your payment method and once the appointment is complete the visit fee will be charged and collected. A transaction may be eligible for a refund up to 30 days from the date of the completed visit. After this period, Zebra Telehealth will not review or issue refunds for captured payments. If you cancel your appointment at least two hours prior to a scheduled visit, the transaction hold will be released, and no charge will be incurred. If you do not cancel your appointment and you do not show up for your appointment, the hold on your card will remain and you will have the opportunity to reschedule your appointment. In the rare event that a Healthcare Provider cancels or does not attend the appointment, you will be eligible for a full refund, or the payment hold will be released. Zebra Telehealth does not guarantee a specific outcome related to appointments scheduled utilizing the Zebra Telehealth platform. If you are unhappy with your appointment, you can reach out to Zebra Telehealth support staff at which time they will determine if a refund is warranted. Zebra Telehealth has the right to refuse service based on patient eligibility.

Zebra Telehealth Terms of Use

The following terms and conditions (“Terms of Use”) govern your access to and use of the ZEBRA TELEHEALTH website (the “Site”), online interfaces, properties, applications, mobile applications, services, and Site materials provided by ZEBRA TELEHEALTH (“Zebra Telehealth”), including related websites and all associated services (collectively the “Services”). For the purposes of these Terms of Use, the “Services” include, without limitation, pages maintained by ZEBRA TELEHEALTH on behalf of third parties and technology integrated into third party websites.  Your compliance with these Terms of Use is a condition to your use of the Site. By accessing and/or using the Services, you agree to all the terms and conditions of this Agreement. If you do not agree, then you may not use the Services. ZEBRA TELEHEALTH’S collection and use of Personal Data in connection with the Services is described in ZEBRA TELEHEALTH’S Privacy Policy.

While utilizing the Services, you may encounter certain Content that ZEBRA TELEHEALTH makes available to you. “Content” means content, text, data, graphics, images, photographs, video, audio, information, suggestions, articles, scheduling availability, guidance, and other materials provided, made available or otherwise found through the Services, including, without limitation, Content provided in direct response to your questions or postings (“Content”). You acknowledge that the provision of such Content does not create a medical professional-patient relationship, and does not constitute an opinion, medical advice, or diagnosis or treatment, but is provided to assist you in choosing a doctor, or other healthcare professional, provider, organization, or agent or affiliate thereof (collectively, “Healthcare Providers”) or otherwise be generally informative.

ZEBRA TELEHEALTH is not a physician or other health care provider and does not provide medical advice. ZEBRA TELEHEALTH is an online interface that is utilized by independent physicians and other health care providers (collectively “Healthcare Providers”) to communicate with you. Your interactions with Healthcare Providers via the Service are not intended to take the place of your relationship with your primary health care practitioners. Neither ZEBRA TELEHEALTH, nor any of its subsidiaries or affiliates or any third party who may promote the Service or provide a link to the Service, shall be liable for any professional advice obtained from a Healthcare Provider via the Service or for any other information obtained on the Site. ZEBRA TELEHEALTH does not refer, recommend, or endorse any particular Healthcare Provider, test, medication, procedure, opinion, or other information that may appear through the Service or that is recommended by Healthcare Providers that may use the Service. You acknowledge that your reliance on any information provided by Healthcare Providers via the Service is solely at your own risk, and you assume full responsibility for all risk associated herewith.

ZEBRA TELEHEALTH does not make representations or warranties about the training or skill of Healthcare Providers who utilize the Service. You will be provided with a list of available Healthcare Providers located in the U. S. based on the information you, your employer or your plan sponsor provide to ZEBRA TELEHEALTH. You assume all responsibility in connection with choosing any Healthcare Provider, whether or not you obtained information about such Healthcare Provide on or through the Services.

  1. No Provider Patient Relationship

THE CONTENT OF THE SITE AND SERVICES, INCLUDING WITHOUT LIMITATION, TEXT, COPY, AUDIO, VIDEO, PHOTOGRAPHS, ILLUSTRATIONS, GRAPHICS AND OTHER VISUALS, IS FOR INFORMATIONAL PURPOSES ONLY AND DOES NOT CONSTITUTE PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, TREATMENT OR RECOMMENDATIONS OF ANY KIND. ZEBRA TELEHEALTH (a) does not employ, refer, recommend, or endorse any Healthcare Providers, (b) does not make any representations or warranties with respect to these Healthcare Providers or the quality of the services they may provide, (c) is not responsible for ensuring that information (including credentials) a Healthcare Provider provides about himself or herself is accurate or up-to-date, and (d) is not responsible for making sure that Healthcare Provider’s’ services are actually provided or are up to a certain standard of quality. We do not control the actions of any Healthcare Providers and Healthcare Providers are not our employees.

  1. Telehealth Video Services

Access to certain telehealth visits may be provided through the Services, including but not limited to, via ZEBRA TELEHEALTH’S telecommunication service, which includes video, telephone, and text messaging. ZEBRA TELEHELTH is not a healthcare provider, insurance provider, or a prescription fulfillment warehouse. In providing the ZEBRA Telecommunication Service, ZEBRA TELEHEALTH’s role is limited to making certain telehealth related information available to you and/or facilitating your access to telehealth services provided by Healthcare Providers. ZEBRA TELEHEALTH is not responsible for such Healthcare Providers’ acts or omissions, or for any content of your communications with them. ZEBRA TELEHEALTH is independent of the Healthcare Providers and does not employ or otherwise exercise any control over the services provided by Healthcare Providers, regardless of whether they utilize the ZEBRA Telecommunication Service.

  1. License And Access

Subject to these Terms of Use, ZEBRA TELEHEALTH grants you a limited, non-exclusive, revocable, non-sublicensable, non-transferable license to use the Services in accordance with these Terms of Use.  All rights not expressly granted to you herein are reserved by ZEBRA TELEHEALTH and there are no implied licenses under this Agreement. This license does not include any resale or commercial use of the Service, or its contents; any collection and use of any Service listings, descriptions, or prices; any derivative use of the Service or its contents; any downloading, copying, or other use of account information for the benefit of any third party; or any use of data mining, robots, or similar data gathering and extraction tools. Site Content may not be copied, distributed, modified, reproduced, published or used, in whole or in part, except for purposes authorized or approved in writing by ZEBRA TELEHEALTH. The license granted to you under these Terms of Use is expressly conditioned on your continued compliance with these Terms of Use.

All of the Content is owned by ZEBRA TELEHEALTH or our licensors and is protected by copyright, trademark, patent, and trade secret laws, other proprietary rights, and international treaties. You acknowledge that the Services and any underlying technology or software used in connection with the Services contain ZEBRA TELEHEALTH’S proprietary information. We give you permission to use the Content for personal, non-commercial purposes only and do not transfer any intellectual property rights to you by virtue of permitting your use of the Services. You agree not to engage in any activity that would constitute copyright infringement with respect to the Site or Site Materials. You may print, download, and store information from the Services for your own convenience, but you may not copy, distribute, republish (except as expressly permitted herein), sell, or exploit any of the Content, or exploit the Services in whole or in part, for any commercial gain or purpose whatsoever. Except as expressly provided herein, neither ZEBRA TELEHEALTH nor its suppliers grant you any express or implied rights, and all rights in the Services not expressly granted by ZEBRA TELEHEALTH to you are retained by ZEBRA TELEHEALTH.

  1. Electronic Communications

When you use the Service, including emails, text messages, and other communications sent to Zebra Telehealth from your desktop or mobile device, you are communicating with us electronically. You hereby consent to receiving such communications for transactional, operational, or informational purposes, and you represent and warrant that each person you register for the Services or for whom you provide a wireless phone number has consented to receive communications from ZEBRA TELEHEALTH. You agree to indemnify and hold ZEBRA TELEHEALTH harmless from and against any and all claims, liabilities, damages (actual and consequential), losses and expenses (including attorneys’ fees) arising from or in any way related to your breach of the foregoing.

Message and data rates from your mobile telephone service provider may apply and are subject to the terms and conditions imposed by your provider. You can opt out of receiving text messages at any time by adjusting your notification settings in your account or otherwise following the given instructions for doing so. Note that opting out of receiving all texts may impact your use of the Services.

  1. Site Access, Security and Restrictions; Passwords

You may only use the Services for lawful, non-commercial purposes. You are prohibited from violating or attempting to violate the security of the Site, including, without limitation, (a) accessing data not intended for such user access or logging onto a server or an account which the user is not authorized to access; or (b) attempting to probe, scan or test the vulnerability of a system or network or to breach security or authentication measures without proper authorization; or (c) accessing or using the Site or any portion thereof without authorization, in violation of these Terms of Use or in violation of applicable law. If you are under 18, you may use the ZEBRA TELEHEALTH Services only with involvement of a parent or guardian.

When creating a subscriber account for the Site, you agree to complete the registration process by providing current, complete, and accurate information as required by ZEBRA TELEHEALTH. If you wish to access certain areas of this Site or the Site Materials available on or through certain areas of this Site (“Protected Areas”) you must choose a password during your completion of this Site’s registration process. You are solely responsible for the confidentiality and use of your password, as well as for any activities conducted on or through this Site using your password. Your access to the Site may be revoked by ZEBRA TELEHEALTH at any time with or without cause. Violations of system or network security may result in civil or criminal liability. ZEBRA TELEHEALTH will investigate occurrences that may involve such violations and may involve, and cooperate with law enforcement authorities in prosecuting users who are involved in such violations. You agree not to use any device, software or routine to interfere or attempt to interfere with the proper working of this Site or any activity being conducted on this Site. In addition to our rights in these Terms of Use, we may take any legal action and implement any technological measures to prevent violations of the restrictions hereunder and to enforce these Terms of Use.

  1. Accuracy and Integrity of Information

Although ZEBRA TELEHEALTH attempts to ensure the integrity and accuracy of the Site, it makes no representations, warranties or guarantees as to the correctness or accuracy of the Site and Content thereon. It is possible that the Site could include typographical errors, inaccuracies or other errors, and that unauthorized additions, deletions and alterations could be made to the Site by third parties. In the event that an inaccuracy arises, please inform ZEBRA TELEHEALTH so that it can be corrected. Information contained on the Site may be changed or updated without notice. ZEBRA TELEHEALTH shall have no responsibility or liability for information or Content posted to the Site from any non-ZEBRA TELEHEALTH affiliated third party.

If you submit, upload, post or transmit any health information, medical history, conditions, problems, symptoms, personal information, consent forms, agreements, requests, comments, ideas, suggestions, information, files, videos, images or other materials to us or our Site (“User Information”), you agree not to provide any User Information that (1) is false, inaccurate, defamatory, abusive, libelous, unlawful, obscene, threatening, harassing, fraudulent, pornographic, or harmful, or that could encourage criminal or unethical behavior, (2) violates or infringes the privacy, copyright, trademark, trade dress, trade secrets or intellectual property rights of any person or entity, or (3) contains or transmits a virus or any other harmful component. Those under the age of 13 may not use the Services.

Zebra Telehealth Refund Policy

To request an appointment with Zebra Telehealth, a valid payment method is required. A transaction hold will be placed on your payment method and once the appointment is complete the visit fee will be charged and collected. A transaction may be eligible for a refund up to 30 days from the date of the completed visit. After this period, Zebra Telehealth will not review or issue refunds for captured payments. If you cancel your appointment at least two hours prior to a scheduled visit, the transaction hold will be released, and no charge will be incurred. If you do not cancel your appointment and you do not show up for your appointment, the hold on your card will remain and you will have the opportunity to reschedule your appointment. In the rare event that a Healthcare Provider cancels or does not attend the appointment, you will be eligible for a full refund, or the payment hold will be released. Zebra Telehealth does not guarantee a specific outcome related to appointments scheduled utilizing the Zebra Telehealth platform. If you are unhappy with your appointment, you can reach out to Zebra Telehealth support staff at which time they will determine if a refund is warranted. Zebra Telehealth has the right to refuse service based on patient eligibility.

The following terms and conditions (“Terms of Use”) govern your access to and use of the ZEBRA TELEHEALTH website (the “Site”), online interfaces, properties, applications, mobile applications, services, and Site materials provided by ZEBRA TELEHEALTH (“Zebra Telehealth”), including related websites and all associated services (collectively the “Services”). For the purposes of these Terms of Use, the “Services” include, without limitation, pages maintained by ZEBRA TELEHEALTH on behalf of third parties and technology integrated into third party websites.  Your compliance with these Terms of Use is a condition to your use of the Site. ZEBRA TELEHEALTH is not a physician or other health care provider and does not provide medical advice. ZEBRA TELEHEALTH is an online interface that is utilized by independent physicians and other health care providers (collectively “Healthcare Providers”) to communicate with you. Your interactions with Healthcare Providers via the Service are not intended to take the place of your relationship with your primary health care practitioners. Neither ZEBRA TELEHEALTH, nor any of its subsidiaries or affiliates or any third party who may promote the Service or provide a link to the Service, shall be liable for any professional advice obtained from a Healthcare Provider via the Service or for any other information obtained on the Site. ZEBRA TELEHEALTH does not refer, recommend, or endorse any particular Healthcare Provider, test, medication, procedure, opinion, or other information that may appear through the Service or that is recommended by Healthcare Providers that may use the Service. ZEBRA TELEHEALTH does not make representations or warranties about the training or skill of Healthcare Providers who utilize the Service. You will be provided with a list of available Healthcare Providers located in the U. S. based on the information you, your employer or your plan sponsor provide to ZEBRA TELEHEALTH. You assume all responsibility in connection with choosing any Healthcare Provider, whether or not you obtained information about such Healthcare Provide on or through the Services.

  1. No Provider Patient Relationship

We do not control the actions of any Healthcare Providers and Healthcare Providers are not our employees.

  1. Telehealth Video Services

Access to certain telehealth visits may be provided through the Services, including but not limited to, via ZEBRA TELEHEALTH’S telecommunication service, which includes video, telephone, and text messaging. ZEBRA TELEHELTH is not a healthcare provider, insurance provider, or a prescription fulfillment warehouse. In providing the ZEBRA Telecommunication Service, ZEBRA TELEHEALTH’s role is limited to making certain telehealth related information available to you and/or facilitating your access to telehealth services provided by Healthcare Providers. ZEBRA TELEHEALTH is not responsible for such Healthcare Providers’ acts or omissions, or for any content of your communications with them. ZEBRA TELEHEALTH is independent of the Healthcare Providers and does not employ or otherwise exercise any control over the services provided by Healthcare Providers, regardless of whether they utilize the ZEBRA Telecommunication Service.

  1. License And Access

Subject to these Terms of Use, ZEBRA TELEHEALTH grants you a limited, non-exclusive, revocable, non-sublicensable, non-transferable license to use the Services in accordance with these Terms of Use.  All rights not expressly granted to you herein are reserved by ZEBRA TELEHEALTH and there are no implied licenses under this Agreement. This license does not include any resale or commercial use of the Service, or its contents; any collection and use of any Service listings, descriptions, or prices; any derivative use of the Service or its contents; any downloading, copying, or other use of account information for the benefit of any third party; or any use of data mining, robots, or similar data gathering and extraction tools. When you use the Service, including emails, text messages, and other communications sent to Zebra Telehealth from your desktop or mobile device, you are communicating with us electronically. You hereby consent to receiving such communications for transactional, operational, or informational purposes, and you represent and warrant that each person you register for the Services or for whom you provide a wireless phone number has consented to receive communications from ZEBRA TELEHEALTH. You agree to indemnify and hold ZEBRA TELEHEALTH harmless from and against any and all claims, liabilities, damages (actual and consequential), losses and expenses (including attorneys’ fees) arising from or in any way related to your breach of the foregoing.

Message and data rates from your mobile telephone service provider may apply and are subject to the terms and conditions imposed by your provider. You can opt out of receiving text messages at any time by adjusting your notification settings in your account or otherwise following the given instructions for doing so. Note that opting out of receiving all texts may impact your use of the Services.

  1. Site Access, Security and Restrictions; Passwords

You may only use the Services for lawful, non-commercial purposes. This Agreement and any supplemental terms, policies, rules and guidelines posted through the Services, each of which are incorporated herein by reference, including the Privacy Policy, constitute the entire agreement between you and us and supersede all previous written or oral agreements. The failure of either party to exercise in any respect any right provided for herein shall not be deemed a waiver of any further rights hereunder.
